iPhone app that's submits details to computer

Hey all,

I own a company and I hire 5 agents who take orders for me.
I need a pretty simple iPhone app that will have 4 fields.
once the fields will be completed the app will email a form containing these fields.
Something like this (Same fields):

(Company LOGO)
Coustomer Name : _______
Name Of Fabric : ________
Length : ____ METERS
Notes :_______________________________
_______________________________
(SEND button)

Once I hit the SEND button I want the app to generate an email to a specific permanent email address with all the details I'll provide.
I need the app to be in the Appstore as well if it's possible.

I hope i managed to explain myself.
it's my first contest so be merciful :)
