Image and video uploader

Project Description:

I don't have time to waste as my past projects that I posted.

THIS IS WHAT I WANT:

A form that gives the user the opportunity to make his own "How-to" tutorial.

Parts of this form:
1) "Personal Information", where the user will be able to write their Name, Email, Age, Country.

2) Which format will be the "How-to"? (Video or photo) and write the TITLE of his "How-to"

3) If its PHOTO version; The user will be able to add a description of STEP 1 of his "How-to" and upload an image of that step. When done, the user must be able to add another STEP. And so on simultaneously until he finish the "How-to".

As you can see, the highlighted box will be duplicated for the next step.

4) If its VIDEO version; just give the user the opportunity to submit ONE (1) video.
