Contact Form + 4 Various Forms

Project Description:
Hi,

We need help for the coding/scripts of our web site (Dreamweaver and Flash used; template used). We have a windows hosting plan.

We need:
1) the coding for the contact form of the website. The information details provided by the user should be sent to an e-mail address. You will find below the coding we currently have for the contact form. You will also find the html page in the attached file "forms.zip".

2) a few forms to be created whereby the client can enter information and the information is received by e-mail (the forms should use the templates of the attached file "forms.zip").
