Shift Bid Application: ASP 3.0 & MS Access Database with Javascript field verification

My bussiness has regular shift bids for my employees where the shifts and number of workers needed per shift change every bid. Because of these changes with every bid, I need an ASP application that is flexable.

Requirements:

1. Must be in ASP Clasic 3.0 with MS Access Database and javascript form field verification.
2. Each bid would need a bid id (maybe date of bid) so it could be made active for bidding or inactive (not shown) and have the associated shifts available and number of workers needed per shift.
3. The bid form would need to list shifts (ex. 9 - 5pm) and workers needed (ex. 30)(this is only for the users informational benifit) for the active bid with a field next to each option for the employee to enter a number to rank their shift preference from 1 to the total number of shifts available. It would simply look like this...

The form also needs to have client side javascript verification to ensure the user enters a number for each field, and it also needs to sum all the numbers entered in all the form fields to ensure they add up to the correct amount based on the varying number of choices before the form is submitted. Example, if there are 5 shift options available then the total should be 15 (1+2+3+4+5). This will ensure the user does not have multple choices ranked the same. The form will also need to submit the users windows nt logon username with the form data.
4. The data from the form needs to be submitted to a seperate table in the database so I can see the the bid id, date submitted, time submitted, nt logon usernames, and all their choices and number rankings so I can work out who gets what shift for every bid.

Budget for this project is around $50 USD. Please only bid if you know what I want based on above and you can deliver within 5 days (I had a bad experience in the past with outsourcing that took months and money with no results ever seen)
