310352 Convert JPG to editable PSD

I am attaching the file. I need this converted to a PSD format so that the pricing can be changed at a later date. Does not have to be the exact same background, but have similar information. Complete redraw if necessary is fine. I just need to be able to edit pricing and text as needed and have a space to add name, logo, address, etc. When completed I need the ORIGINAL PSD file with all work, fonts, etc to edit later.
