Brochure - Form - Template

Need a template or form preferably in word doc or whatever you recommend for two separate documents with three columns list with check mark boxes on the left side next to each item. Top of the page is the general information section and right below (around 2 inches lower), the columns starts. Subsequent pages will include only a small text on the top and columns right below. There may be an overlap issue due to the some of the text length and some pages may have to go with two columns instead of three. It's an easy job and we pay $30.00 firm. There are total of two forms with multiple pages. Sample is attached. Turn-around time 24 hours.
