Access Database
We would like a database created in Microsoft Access relating the following information.
Table 1:
• business name
• owner's name
• manager's name
• Address
• booth number
Table 2:
• business name
• question
• answer
• booth number
Entry Form to accommodate these two tables above.
Reports 1:
• Titled Expo Questionnaire
• Ten unique questions on each report
• One question per business up to 10 questions
• Questions offset to create eight unique reports of 10 questions
o Example: (B=Business, Q=Question)
 Report A: B1 Q3, B2 Q4, B3 Q5, … B10 Q2
 Report B: B11 Q4, B12 Q5, B13 Q6…B20 Q2
 etc
Report 2:
• Titled: Answer Key
• Ten unique questions on each (correlated to Expo Questionnaire format above)
• Answer
