RECREATE WORD DOCUMENT FROM PDF TEMPLATE AND ADD PDF FORM FIELDS

Intelligent provider needed for some Word and PDF work. We will provide you with 3 different but similar 1-2 page PDFs and the original .doc/.docx file for one of the PDFs. Your job is to create a .doc or .docx of the remaining two PDFs exactly as the original, including fonts and formatting. In addition, you will be responsible for adding form fields to the finished PDFs so users can type information into those PDFs.

PROVIDED:
-1 original of 1 of the 3 PDFs in Microsoft Word format.
-3 finished PDFs
-Font pack for font used

PROJECT REQUIREMENTS:
-Create Microsoft Word document replicating the original of 2 of the PDFs
-Add form fields to the 3 PDFs to allow for users to type the information in and print the document.

DELIVERABLES:
- 3 PDFs with form fields. All input locations.
- 2 Original Microsoft Word documents.
