Project Description:
I have a fully functional website. Currently I have a contact form that works but does not have a custom skin and it is formatted badly. I want the case evaluation form (contact form) to be made to look a lot like the sample form I've attached. The sample form has a 5 step process, however, my form is only 3 steps. I believe the form I have is programmed in jquery, in case you need to know that.
Also, I want you to reformat my current wordpress layout (only to be used for a select # of pages) to have the sidebars the same as this other website (See Attachment).
I need this done in the next few days.
