Hi,
I need an icon for an aviation app on iOS. Here are my ideas:

* I want it to hold a document, your artistic interpretation of http://www.lfv.se/AIP/AD/AD%202/ESSB/ES_AD_2_ESSB_5_3_en.pdf
* I want it to offer a feeling of depth, like this icon http://dribbble.com/shots/150614-Boxing-Glove-iPhone-Icon
* It should look like a real item, like this icon http://dribbble.com/shots/349320-Camera-iOS-Icon
These ideas should just give you a general idea of what i want, please don't feel locked to the colors etc above. The only thing that is required is that you present the document linked above on something that looks like a pilot's kneeboard, with the general design feeling given by the example icons.
I also want the PSD well organized with named layers, and delivered with the project. The icon should look nice and crisp in 1024x1024, as well as in all the standard iPhone and iPad icon resolutions (57px, 72px, 114px, 144px).
