Upload Form isn't parsing Files

Project Description:

I need someone to look at the Upload Form on my webpage and fix the problem in which it's not uploading the specified file to the website, it's only refreshing the upload page. When completed, the page should display the results of the uploaded file properly.

Statistically this upload form is already set up to display the results, however, by tinkering with it you may mess up what it's supposed to do and what it's set up to do already. If this is the case you will be responsible for correcting it.

"Upload_Process1.php" has been renamed only to post to Freelancer.com. It wasn't letting me post with the name "Upload_Process.php", which is what the file is actually named.
