Project Description:

Seeking a C# coded web form with USB card reader integration. Finished product should be a web form consisting of the following fields:

Drivers License Number
Title, First Name, Middle, Last Name, Suffix,
Address 1, Address 2, City, State, Zip, County,
DOB, Sex, Height, Issued Date, Expiration Date

Web form can have Java, Flash, ActiveX controls/elements embedded in the page.
Swiping a valid Florida state drivers license through a locally attached USB card reader will populate the form with fields automatically based upon available fields on state drivers license.

Ultimately, this code will be heavily modified once complete to be integrated into an existing application. Code must be clean, commented and logical.

I realize that you cannot guarantee compatibility to all types of hardware/card readers. I am seeking bids from individuals who have already designed such an application, and have a known method of accomplishing this with definite success with a particular brand/model of readers. In other words, I will allow you to dictate what hardware will work with your code.

Likewise, this product CAN be tested with a drivers license of a state other than Florida. This application ultimately needs support for ALL states. Your code should easily be modifiable to encompass support for these states / id cards.
