Automatically generated email contact form

Project Description:
Hi everyone,

I require a webpage that builds a contact form based on my requirements...a form that I can then upload to my clients hosting servers...

1) I need the option of outputting to two types of forms, one in php and the other just in js (just in case my clients server doesnt support php)
2) The form must have the option for me to specify any number of form fields e.g. Name + Address + Email + Subject + user specified + user specified etc etc
3) I need easy to follow instructions on how to upload to my hosting servers, how to format the form to suit my clients design requirements
4) I need an option to protect my clients email address from bots/spam with captcha (http://nfriedly.com/techblog/2009/06/how-to-build-a-spam-free-contact-forms-without-captchas/)
5) Once the form has been built - the webpage should generate code that I can copy and paste into the js/php & html files or a jsphp & html file that I can then use...complete with instructions on what to do..
