Google Map Android App

Need an application that has one view, a google map. This map will ask the user if they can have their current location, display a map and the map will have markers on it that are automatically downloaded from a mysql database. If the user does not allow their current location to be seen, the map will automatically zoom out and allow the user to zoom in appropriately, markers will be shown at lower levels of zoom only. Users can contribute a marker by pressing a button or otherwise select from the menu to "Add a Point". A form will come up with the users current coordinates, and will request an email address, title and description of the point. Upon submission the point will be sent to the database. Bare bones app, no graphics needed.
