Simple Blackberry Application

I need a simple blackberry application.
All it needs to do is:
1. First time the app loads, show a setting screen. User type in a URL, email and password. Information saved to the phone memory and proceed with #2.
2. If the app starts and info does not exist in memory go to #1. Open URL and pass email as parameter + an MD5 of the Password in a browser window. So this would be typically:
url?email=&password=md5password
3. Give the user the option to exit the app, or open settings to save new values.
That's it. I would prefer if the browser window can be embedded in the application as oppposed to using something like platformRequest(url...) that opens the phones native browser as seperate instance.
I need the source code and it must work on all blackberry 4+ platforms including touch screen phones.
I will provide a simple png for a 3 second splash as well as an icon for the application.
