51712 Convert Excel spreadsheet to online form and database

I need the attached excel spreadsheet made into an online form that will record the information into a database. All the calculations that the spreadsheet does need to be retained for entry into the database. Will also need online form to be able to pull simple reports from the database so reports can be printed if needed - displayed much the same as the Excel spreadsheet would present a printed copy. Prefer asp and Microsoft access but can also use PHP and mySQL if it is easier to complete. Please post questions after reviewing the attached spreadsheet and thank you for considering this project.
