Hello,
A small project is ready for a frelancer.
We would like a simple form built in HTML or PHP/ASP with some fields to enter data into, and also to be able to attach files.
When everything is entered customer should press "send" and everything should be e-mailed to our e-mail and a copy to the submitters e-mail.
If possible we would like to add a auto generated reference number to everey submition.
The form should look like this:

So, hope someone is up for doing this little project...
