Change html based on user answer- multiple pages

This should be a pretty easy job for an experienced php programmer. I already had a php programmer add a way to modify html content based on user radiobutton answers. And it works but only for 1 page. 1. I want it to support as many many pages as I put the questions on - so they should not step on each other. 2. User answers on page one are retrieved on page 2 and different html displayed based on the answer retrieved (yet, page 2 should allow it's own question to be asked. 3. Lastly, I have an opt-in form. I want the NAME from that form saved as a cookie and retrieved on the next page. Your code should be well tested, and documented. And you must be willing to anser questions about your work. - If no name is found, the default name 'achiever' should be used. It should work with firefox, ie, and netscape.
