Church Database simple entry form

What I am trying to do is to have a very simple PHP entry form to post into a MySQL database one record at a time from a web browser. Users should not be able to create new records, only modify existing ones. They should press submit to enter the record they have modified, "next" to scroll records forward and "back" to scroll records backward. I need a way to limit where they will start inputting and where they will stop inputting, in other words, I only want to give each user 100 or so records at a time based on my determining their starting point in the database and their ending point. Related to this I will send them different html pages with the appropriate select name from dfwchurches limit 1,100; statement for different html's I will have ready for them to input on. I already have a lot of code . . I'm about 90% done, but I'm missing a few things. Attached are 4 files which should give full descriptions.
