Customized Feedback Form Wizard I want a small software package that creates a feedback form for a website. This will allow clients to put a Feedback Form or Contact Form on their site so that visitors can send messages / email or make comments using PHP. I would like it to produce the php script and also the html script for embeding on the webpage. I would also like it so that clients can add various options to gather different optional amounts of information. This software will be strictly for novice so it needs to be simple to fill in and execute. The main use is for running feedback forms to do market research however if we can also allow for optional fields then this would also be desireable. It must also have the option to redirect to a thankyou page. Finanly all the information returned to the client must contain the information filled out in the form. A simple example of this can be seen at <[login to view URL]> The problem with this script is that the name and email is not shown in the returned email and I know this can be done easily by adding a little code. Idealy this project would be compiled in a simple yet business like software package that resembled Xp in looks. The final software product would be excuted on a windows platform. The customers would then be required to fill in various options in fields and then a button to create. This would then produce a PHP file and the corresponding html file to embed in their web page. Quentin. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform All major windows platforms for the software and general PHP for the final outputed results.
