Lead Capture Page for MLM site

Project Description:
I am needing a lead capture page developed. The lead capture page will link to my company web site, so I am only needing one page.

The site is for a health and nutrition MLM company called Amigo Health. You can see the company site at:

I can supply you with several graphics to potentially use on the lead capture page, but I would also be VERY open to someone with graphic design skills to create a header graphic or other graphics on the page that makes the site appealing and attractive.

I also have 3 flash audios that can be incorporated which you can find on the second URL above.

The lead capture page will need to capture their name, phone number, and email address. I will be using Aweber for my autoresponder. Once they enter their information, they can be redirected to the first URL above.

There should be an equal balance on the page between the health benefits and the business opportunity. But, I want to try and avoid any of the "hype"-type language that can be a big turn off (if you know what I mean).

This should be a very simple project if you know what you're doing, but I am willing to pay extra at the end of the project for someone with proven graphic skills and who can create an excellent landing page.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.


2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

This lead capture page will be hosted on my Hostgator Unix account.



Ideally, the page should be targeted toward IE and Netscape browsers, and would be ideal if it worked for Mozilla too.
