Embedded audio player/ Flash player for website REPOST

Project Description:
***THIS IS A REPOST PLEASE DO NOT BID IF YOU CANT GET THIS DONE ASAP***

We would like to have a custom Flash Audio Only Player for our site. As of now we use the free google .swf player but need to have one made just for us.
See the files of the OLD.jpg and NEW.jpg look of the player skin. I have also included the php file that the player is called from.

List of must haves.
1-NO Audio Player with any advertising on it or in it.
2-NO Labels on the player. (i.e. GOOGLE)
3-Start volume set to 5 on a scale from 1-10 (50%)
4-Lables above the player as they are on the NEW.jpg
5-Colors as they are on the NEW.jpg
6-Must work as it does now with the names of the files. SEE php file

Please tell me your bid, how long it will take, and how many revisions you allow.
No money we be released until all file are in hand and have been tested on are site.
