Project Description:
I need a customized flash contact form which once confirmed, submits the form to my email address and sends a confirmation to the user.
I want this form to match the look of my site, which means the form needs have a creative edge.
My site is flash so I would need the form to be done in flash calling a php script.
This bid will include the flash form design, script installation and Go-Live of the fully functional contact form on my server.
As this is a very simple project for someone who deals with php scripts, I expect the bids to be rather low. You will need to have graphic skills since the form will have to match the look of my site.

I want to see examples of PHP contact forms done in flash only.
