Video capture with webcam on web site

Project Description:
We are looking for a solution for our web site that enables users to record a video with their web cam and saves the file so that the files can be viewed by the site administrator. The solution must work without all browsers and without special add-ons.

I added 2 pictures of what we would like to get:

video1: visitor sees the live picture of his web cam, when he is ready to start recording, he hits the REC button (time 90 sec starts countdown). When the visitor wants to stop recording he hits STOP button ( if STOP is not hit, recording is automatically stopped in 90 seconds). Visitor adds his name and e-mail and phone number and clicks SEND.

video2: on the admin side, we can see all the recorded videos that were sent (with name and contact information). Videos open in a lightbox.
