I have an existing website in development, and the current registration form is quite ugly. (See file attached) I am looking for someone to make a much better looking form and integrate it with the backend mysql database that already exists. Please know how to both design and implement a nice looking form. I am using Rackspace to host so please be able to navigate that environment. php, html, mysql, design, sympfony php
