Put exit splash and dropdown optin box on a website

I need a floating dropdown box asking for name and email to appear on the left hand corner the website. I want a box similar to the one on this website: [login to view URL] I also need the graphics to be done for the box. I need an exit splash to be installed on my website so if someone hits the back button or closes the window, they get a pop up message and get redirected to another page where they get ask their email.
