Hello,
My client currently has an Ajax form on his website. This form collects data over a series of 3-4 steps, then submits the data to an affiliate provider.
He would like to capture these details before they are submitted, and send them to a designated email address and also store them in a MySQL database. A simple control panel is required to show these sorted in date order, with an option to download the data.

This should be a quick and easy job for someone who knows what they are doing, so high bids will be disregarded.
Payment will be made by GAF escrow at the end of the project; this is non-negotiable.
