Create an image upload page that will work on a wordpress site. A development site will be created for you to test on if needed.
The page will have a similar functionality to Example
The image uploader script used should be plupload or similar cross browser functionality - you can argue that another script is better suited for the job before starting work if needed, uploaderfive?
The user needs to enter the following data into form fields:

The image upload must allow multiple uploads and work in a similar way to the jquery queue widget example on www.plupload.com. Showing what files have been selected and overall progress.
On clicking "Upload and submit", image uploads are started and then data is submitted. Confirmation of successful upload and submission is displayed to user - error message is displays if not successful.
Images are grouped into a folder on the server titled by "ebay username + phone type + number of images". If the filename already exists on the server the folder will be titled "ebayusernamephonetypenumberofimages-v1" etc.
Images inside the folder will have the ebay username prepended to their titles (ebayusername+originalfilename).
Folder to also contain text/excel file, comma delimited of users data they entered into the form.

Upload must automatically version number image files if filenames are the same - for example if multiple images titled "image-01" are uploaded, they must not overwrite each other.
Upload must work on most browsers and iphone/android devices which is why plupload is suggested.
Upload should block files over 25meg.
Upload should only allow images to be uploaded - ideally non image file should be greyed out of file selection and/or upload should fail, telling the user they can only upload image fines.
CSS styleing of the form is not that important so long it has appropriate selectors to style as needed (similar to the provided example above). Form should be in a functional and usable state without any styles.
