Project Description:
Need a fully functional multi field form (see InsureMe.com's form) that can be deployed as a Wordpress Plugin across multiple Wordpress sites. We currently have several sites powered by InsureMe's co-branded forms and are looking to simply replace their form with our own.

We would like the site to function and flow precisely the way it is now, only the form would be powered by this plugin rather than Insureme's form.
The form must store data in a organized data structure in the wordpress database and upon receiving the lead will forward it real-time to one of several predefined agents based on the leads zip code. The leads will be forwarded in a round robin sequence in the case where several agents share a particular zip code.
Plugin must also include the following features:
Affiliate Management Panel:
Manage agent contact, payment and general account information, define lead filters and manage account balances.
Client Dashboard:
Allows agents to manage aspects of their accounts from an easy to use dashboard. Features include: Ability to monitor current credit balance, make payments to their accounts, update contact information, and select type of leads they want sent to them and to which email address the real-time leads should be sent.

NOTE: This is now the second time we are posting this project. We had it in development with a company here in freelancer that said they could deliver in 5-7 days. 12 days latter we were forced to pull the project. If you are not sure you are capable, or you are not 100% POSITIVE that you can deliver on time... PLEASE DO NOT BID!
