Javascript/ PHP page locker script

Project Description:
What I need done is basically a page locker.

When the user visits the page with the code, a window appears and blurs out the original page.

It will have three links, when the user clicks the links and does the required information on the links, it postbacks and sets a cookie. Once the cookie is placed, the window dissapears and allows the user to go back to the original page.

I have examples. I want it pretty much a clone of the blackhatcode breaker script

I pretty much want the same thing but with a few extra features.

I have a budget of about $200. I kind of need this script fast, but will allow a good bit of time.

This script will just be for personal use on my websites, so if there could be an included license system, thatd be great!
