Project Description:
I need the attached form to be converted into an online PHP form. I would like the results to be emailed to a designated address in PDF format. Everything must be very organized and I would prefer that date selectors are used rather than having the end user have to manually type the date in. In addition, under "away games" I would like to have a "+" sign that would allow the user to add an extra row(s) if necessary. There will also be "required" fields which I will specify once I select a bidder. I need to have this completed within the next 15 hours. If you have any questions at all, please ask before you bid.
