Project Description:
This project is for the creation of an online application (from scratch).
The application is similar to an online diary and booking solution - capturing a person's activities against a date and details of where those activities are to take place and the cost involved in performing those activities.
The application requires up to 20 forms needing to be designed using PhP& MySQL.
Mainly user input forms, and some search/results forms.
EG:
Activity create/update form.
Add new user form
Add new activity form
Add new "place of activity" form
Summary form showing activities listed by date and filtered by activity type.
Basic menu (repeated on each form) to choose relevant user input form or search form.
Basic form design only - no fancy layouts or web design at this stage, but enhancement of look and feel maybe requested as a phase two project.
Clean useable forms black text on white background is fine with basic CSS (e.g. h1, h2, h3, input, font).
IMPORTANT: Best practice for secure forms design is required - use of MySQL prepared statements, anti-sql-injection, etc
Coding must be well documented.
LAMP environment.
Database schema is already built, but can be changed to suit solution design if needed.
Any bidders must demonstrate previous experience or competency.
