Project Description:
I have a Joomla 1.6 website www.riskwave.com.au which I would like to add a self assessment / checklist form(Survey) to. I would like potential clients to be able to complete the checklist, eg radio buttons for yes, no answer & then the result be automatically calculated eg 8/10 or 3/10.
Then the result be forwarded to me if they submit a query & choose to send me their completed checklist. Also if the client & I could print a printer friendly version of the results which has a company logo on it.
Attached is a Microsoft Powerpoint document with more detail information.
