Hi,
I need a jQuery Mobile expert who can do a simple HTML5 page that can display itself on iPad, iPhone, Android tablets (9.7", 11").
I need a simple and decent looking design that has the following:
1) To use our logo at the top of the page
2) 3 questions with 5 radio buttons
3) 1 question with 1 textfield
4) 1 question with 1 textarea
5) 1 submit and 1 reset button
6) Simple validation to ensure the fields are entered
Upon pressing Submit, the page will do an json post to a PHP file with all the fields and will return a success (or error message). Upon success, it will redirect to another page that says Success.
Please attach some project portfolio for HTML5. I believe this to be a straightforward project, but if you see the need to quote more than $250USD, please go ahead and I will consider it. I have a few more of bigger HTML5 projects and will look to work further on other projects with successful winner after demonstration of ability and good communication on this project. Thank you
