I have an application that creates (Adds) tickets and saves them to MySQL database. I want to expand the ADD ticket function to also offer the option to email the form to a list of user(s), either with TO: and/or BCC:. I have a table with staff name and their email address. What I want to happen is, when the user creates a new ticket, on the STAFF drop-down box, if they want to send an email to that user, they click a button, either To: or BCC: and add it to another text box. When the user is done and ready to submit (button on the bottom). The form will save to the database (just like it does now) but ALSO sends an email with the data in the form to the users listed in the text box with to or bcc respectfully.

[login to view URL] is what it looks like now and works.
[login to view URL] is what I want it to look like when you are done.
We will communicate by emails or AIM, no phone calls. I need this by Monday May 28th or Tuesday the 29th. You will have ftp access to my test server and you will have access to phpMyAdmin to see the tables if necessary.
