Filtering MySQL database query using html form (checkboxes, drop down lists, text fields)

Project Description:
I can successfully fetch all records from the database, but now I need to filter the results, using an html form with check boxes, drop down lists, and text fields. If the user leaves any of the input fields blank, I obviously don't want the results to be filtered by that particular input field. I also need the form to be sticky, and remember the checkboxes, selections, and text input fields, after the form has been submitted. I already have the form built, I just need the PHP and mysql query to make it work correctly. I want to show the first 25 results and have the rest of the results paginated. I also need to be able to sort each individual column by clicking on the column title. Please read all the comments in the attached file, I commented on every form element, and on the mysql query.

I forgot to add one thing. 1) When the results are displayed from the database, I need the players name to be a link to that players page. I already have all the formatting for the player page done, you are not required to do that. I just don't know how to make his name a link to that page, and I need all his data from the database too, so I can display it on the page.
