I need a web form created and below are the results I want:
*Create a login page.
*Create an admin page to allow new user creation with rights as "user" or "admin".
*Upon login admins should have the following options: Create New User, Search and Edit Users, Delete Users, access to Create New Form, Search for Client, delete forms
*Upon login users should have the following options: Search for Clients and Create New Form.
*Search field should query previous added information by "Phone Number' and,or "Name".
*After completing form "Submit" should add to a database and some of the custom fields imported to another document for printing.
*Logout field on top.
The current form have fee fields which I would like to calculate and place total into another field. Form can be altered. I need a notes field added to current form with auto dates for track notation purposes.
