488710 PHP file upload

I need a php script that uploads pdfs to a directory. Provide checks for filesize and filetype. Make variables at the top of the script that configure allowed filesize, allowed file type and the upload directory path. For the display page, we need a form. The form will have the file upload and a text field called "file name". Under the form, links to download each of the files will be displayed. To the right of the link will be a delete link. The file names will be stored in a mysql database table called "pdf". The table will have three columns: id (int), filename varchar(100), linkname varchar(100).
