Wordpress custom dynamic form - repost

I have a Wordpress site that contains 2 sections. 1 section allows registered users to post their own 'Opportunities' which turns into a list that displays on a page. The 2nd section is a form that has First name, last name, subject, message. On this form, I need to pull the Opportunities for that specific users into a combo box.
This should be simple for someone that knows php, Wordpress custom Plugins, MySQL, and Javascript/HTML/CSS. This should not take more than 2-3 hours to do.
