Project Description:
I have 2 forms i need made into web forms and when the form is submitted it stores the data in the database and also has a way to click a button to save as a pdf file with the same look as the original with data put in it. I am attaching the 2 pdf forms. This is to be website based and not pdf input.

payment in escrow and released when done.

i need this done asap. i woudl like to choose someone today.
