226474 Form to input into database

There are 2 parts to this project. 1) I need a bit of code I can add to the bottom of our bulk newsletter email that will hyperlink to a form and forward the sender email and recipient email. This information is already being used in the email template by using variables like [email]. 2) The form is attached that I want the link to populate. When the "Send Email" button is clicked, the information needs to be added to a database. The database is already in existence, so this information needs to have it's own table.
