I need the SAME simple form installed in 2 places on the same website.
The form results are sent by email to the owner of the website, and a "Thank you" email must be sent to the visitor who filled out the form.
The form is aready on the site; just attach formmail.php script.
NOTE: WE PREFER TECTITE.COM'S FORMMAIL.PHP. IF YOU USE A DIFFERENT FORM SCRIPT, YOU MUST GET OKAY FROM US FIRST.
