Type: Mix of Textboxes, Checkboxes, Option boxes (Stars Rating), Dropdown list, Comments
Results: Bar charts, Stars rating comparison vote (best)
Results of survey:
MySQL database: Approx. 5 tables, about 35 fields
Visitors’ access types: Guests, Free (Restricted access) / Full Member (Unrestricted access)
