Project Description:
A web app which allows call centre users to record the reasons for calls from a selection of responses setup in the admin. These submissions would be stored in a data file which can be extracted as a cvs using a date filter to create reports on the call centre calls.

Our call centre has many callers per day and we currently manually collect information about the reason for the calls so we can work out how to improve our business. We want a web based form /app that presents a number of options, each with their own sub options for the user to quickly input reasons and details for a call. At the end of the process the user would hit save and the data would be saved to the data file with a timestamp, all the user selections and the operator name. The data file would be stored on a network drive where all users would save the data to.
We would require the ability to add/remove/edit options at each level along with add/remove/edit operator names.
