Project Description:
We require a PHP form with an email to admin feature similar to that of the functionality of the registration form located on www.SolarisCredit.com and we need to add and incorporate an SSL layer to the website with this form.
Requirements
1) Form must have an email to admin feature.
2) Form must have backoffice feature for admin to view registrations.
3) Form must use our SSL certificate.
4) Form must provide fields to collect personal data, billing data, and data collection from our 3 services (Credit Repair, Shelf Corporations, and Corporate Tax Prep).
