Acrobat Fillable Form

Project consist of creating a PFD Fillable Form (acrobat editable). The data is taken from a excel workbook.

The Fillable form must to be able to auto populate fields as data is entered. The framework and form outline is mostly complete. Java-scripting will need to be created and implemented.

Form will be used from a webpage and iPad.

I end result will be for users to complete the 'product request' form and submit via email or print.
