Project Description:
Dear all,

Hereby an interesting project for you to bid upon.

Background: We are a small hospital who have patients that undergo a MRI-scan. Currently there are no digital records of the MRI and their request and execution date. We currently manage this with handwritten registration
forms. The main usage of this application in the initial stae will be to construct a database of the patients who
received an MRI scan and to be able to run basic reporting.
Later on, ideally after completion of this project, we will be requiring an additional module called research report. This will be a form with which an authorized user can compile a report of the MRI scan. Since this is a comprehensive module on its own, we would like to develop this later. Keep this in mind while programming the application.

Since this is a webbased solution we would like you to use PHP and MySQL.

Note: The entire application should have a header with logo's and a footer with a copyright notice. These logo's and copyright notice should be editable and shall be provided upon implementation.

Timelines:
Sunday, 27th May - Mockups of the forms. Preferably online with active fields to get a look and feel. The project will be selected after the first mockups are provided or any previous online application designing project is shown. The look and feel is an extremely important part of this application.
Monday, 28th May - Protocol registration module ready for testing. (Payment 10%)
Tuesday, 30th May - Patient registration module ready for testing. (Payment 10%)
Thurday, 31st May - User and permission management module ready for testing. (Payment 10%)
Friday, 1st June - Reporting module ready for testing. (Payment 10%)
4-7 June - Incorporating our feedback and a final testing round. (Payment 20%)
Friday 8th June - Project completion. Implementation on our server. (Payment 40%)

We are very strict regarding the timeline. Please provide an accurate timeline if the aformentioned timeline cannot be met. You can assume that there shall be no delays on our end.
