Make changes to existing html website

Hello,

I need the following changes made to an existing website.

1. Home page - Add slider with 3-4 for rotating images (Remove existing video) use this slider: http://projects.craftedpixelz.co.uk/craftyslide/

2. Free Quotes page - Add a drop down selection above "your message" (see attachment)
3. Services >aeration page - add video from home page to this location
4.Add additional portfolio page to site titled " Before & After"
5. Menu tab - add "Before & After" to top and bottom menu on all pages

That is it.. Please see attachments for details.

ALSO, We are looking for long term partners that can cleanly code sites quickly in css,php,html5
I will provide you the design in PNG format and you slice and code my projects.
