Mobile phone Unlocking script made

I need a script made so i can sell unlock codes for unlocking alcatel phones, each code will cost £1.99 and the clients will pay by paypal. I will need to collect the imei number, and provider id. I will also need the make of the mobile phone which clients should be able to choose from a list of phones we can unlock. There should be some kind of gallery where i can add the pictures of phone we can unlock.
