PHP page to upload csv file

We need a nice looking PHP page to upload 6 CSV/Excel files and load it to 6 MYSQL tables i.e. one for each file and it should have button/buttons to export data in csv format from another 6 MYSQL tables into 6 files i.e. one file generated for each table. Simple. We will be paying $10 for this 6 file project and want it to be completed for first file today itself. This is a simple import / export feature and shouldn't take long for a PHP expert. Also please note freelancer.com is yet to transfer our funds from [login to view URL] so we will not be able to create any upfront milestones or make payment before work is done.
